This program provides an overview of the management of patients with advanced heart failure at the end-of-life. The focus is on measures to support the extensive symptom burden present in this population, while promoting quality of life.

Categorize patients as eligible vs ineligible for hospice care based on current heart disease state, history, and prognosis.
Interpret patient symptoms and match to the appropriate level and location of care.
Match common end of life symptoms for patients with heart disease with the appropriate pharmacologic/non-pharmacologic measures used to promote comfort.
