Exploring Social Determinants of Health for Clinicians

This course shows the home health and hospice clinician how to identify modern social determinants of health as they relate to updated Healthy People 2030 Goals. Realistic examples and scenarios will allow clinicians to learn how to avoid implicit biases, reduce health inequities, and develop individualized, appropriate interventions for all patients.

Define the terms social determinants of health (SDOH) and healthy inequity.
Identify at least 5 common SDOH encountered in everyday patient practice.
Describe the 5 Healthy People 2030 priority areas related to SDOH and inequity reduction.
Explain how to avoid implicit bias by identifying stereotypes, performing self-reflection, and preventing discrimination based on SDOH.
Apply realistic patient care goals and individualized interventions related to the 5 Healthy People 2030 SDOH priority areas.
